CHOC Health Alliance is an open access plan so you do NOT need a referral from your pediatrician or any other entity to make an appointment.

Cash Fees

If you do not have insurance, or have an insurance that we are not contracted with, you can use our cash rates. 

New Visits: up to $255  

Follow up Visits: up to $197

Destruction of Warts/Molluscum:  

Biopsy & Surgery Rates Vary. This will be explained to you prior to Scheduling your procedures.

On The Day of Your Appointment:

Please bring your insurance card and your ID with you.

Co-pays will be collected upfront. Please be prepared to pay. We accept Apple Pay, Visa, Mastercard, cash and checks.

As each insurance policy is unique, it is ultimately your responsibility to verify with your insurance prior to your visit whether or not our services will be covered/paid for by your insurance company or will be (partially or fully) your responsibility to pay. 

After your insurance company has processed your claim, you are responsible for your bill. We will send you 3 statements and if we do not receive a payment your account will be sent to the collection agency. 

Cancellation/No-show Policy:

In order for us to see you in an efficient manner, we depend on our patients to be prompt and to notify us 1 business day in advance if there is any need to reschedule. 

We will charge a no-show fee of $50 if an appointment is not cancelled 1 business day in advance. In addition, if a patient arrives more than 15 minutes after the appointment time, we will need to reschedule.

HMO Plans:

Needs Prior Auth before scheduling

Out of Network Insurance:

If we are not contracted with your PPO insurance, we encourage you to contact them directly to see if your plan provides out-of-network benefts.

Cash Patients:

If you do not have insurance, or have insurance with which we are not contracted, our office offers reasonable cash rates for new and return patient visits. If a biopsy or procedure is performed this may be at an additional charge. Any charges will be explained prior to performing any procedures.
